Check your benefits before you book

Ask the practice and your insurer:

  • Is Lauren participating in my particular plan?
  • What is the cost for the appointment I need?
  • Does my plan require a referral or prior authorization?
  • What deductible, copayment, coinsurance, or visit limit applies?
  • Do I have out-of-network chiropractic benefits, and how much of that deductible remains?
  • What percentage or amount is reimbursed, and what documents and filing deadline apply?

Village Medicine’s new-patient guide explains its general billing approach. Confirm the details for chiropractic care with Lauren before relying on coverage.

How to submit a superbill

  1. Check your plan. Ask your insurer about out-of-network chiropractic benefits and its claim-submission requirements.
  2. Pay for your visit. Confirm the amount due and payment arrangements with Village Medicine before your appointment.
  3. Request your superbill. Lauren will provide one for your out-of-network claim. Check that your details and the amount paid are correct.
  4. Submit your claim. Follow your insurer’s instructions for its member portal, claim form, or mailing process. Attach the superbill and any other required documents before the filing deadline.
  5. Keep copies and track the result. Save the superbill and submission confirmation. Your insurer determines whether the claim is covered and how much, if anything, it reimburses. Contact your insurer if it requests more information or you have questions about its decision.

Superbills explained

More than a payment receipt

A superbill includes the provider, dates of service, services and charges, and procedure and diagnosis codes your insurer uses to process a claim.

Keep a copy along with your submission confirmation. Reimbursement depends on your plan’s out-of-network chiropractic benefits.

Frequently asked questions

Can I use Kaiser insurance?

Lauren is in network with Kaiser PPO plans only. Confirm your specific plan before scheduling.

Will I receive a superbill if I’m out of network?

Yes. Lauren will provide a superbill for you to submit to your insurer. Your plan determines any reimbursement.

Does accepting insurance mean I pay nothing?

No. Your plan or claim determines coverage. Confirm any deductible, copayment, coinsurance, and billing arrangements before your visit.

What is a superbill?

A superbill is an itemized receipt prepared for insurance. It includes the provider, dates of service, services and charges, and procedure and diagnosis codes. You submit it to your insurer through its out-of-network claim process.

Will I definitely be reimbursed?

No. Reimbursement depends on your plan’s out-of-network chiropractic benefits, deductible, and other coverage requirements. Lauren provides the superbill, but your insurer determines whether and how much it pays. Check your benefits before your first visit.

Can I submit a claim if I pay at the visit?

Yes. Request a superbill showing the amount you paid, then submit it with any required claim form through your insurer’s out-of-network process. Keep copies and follow your plan’s filing deadline. Submission does not guarantee reimbursement.

Do I need a referral or prior authorization?

Requirements vary by plan and injury claim. Ask your insurer and Village Medicine whether your appointment with Lauren requires a referral or prior authorization, and confirm any requirements before scheduling.

What if I was hurt at work?

Lauren is a Washington State Department of Labor & Industries (L&I) provider. Tell Village Medicine about your work injury when scheduling and ask which claim details, records, and authorizations it needs. Confirm billing arrangements for your particular claim. Learn about work-injury visits.

What if I was in a car accident?

Lauren accepts all auto accident and personal injury insurance. Tell Village Medicine about the accident and any existing claim when scheduling. Ask what documentation to provide, how billing will work, and whether any costs may be your responsibility. Learn about care after a collision.

Can I use FSA or HSA funds?

Ask your FSA or HSA administrator whether your chiropractic visit is eligible and what documentation to keep. Confirm accepted payment methods with Village Medicine before your visit, including whether you can use your FSA or HSA card directly or need to request reimbursement from your account.
